Campus Optics Pickup Request Form (updated 9/21/26)
All hazardous waste pickup requests will be transitioning to our Campus Optics platform. You can submit a pickup request by clicking the following links, filling out the form, and submitting your pickup request. SSO login is required.
If you are having issues submitting a pickup request via the Campus Optics platform, please use the temporary workaround method listed below to submit your request.

Hazardous Waste Pick Up Request Form Alternative
Please follow these instructions to submit a Hazardous Waste Pick Up Request if the primary forms are down:
- Please complete the Word document linked below. Enter chemical information as usual.
- Attach completed form to an e-mail addressed to hwm@ehs.ufl.edu
- Additional information (chemical lists, label information or images) should be attached to the same e-mail.
