The Hazardous Waste Pickup Request Forms require GatorLink ID login. If prompted, on the Microsoft account screen, log in as shown below:
GatorLink Username@ufl.edu
On the UF Authentication screen use:
Username: GatorLink Username
Password: GatorLink password

- Pick-up form must include all hazardous waste to be collected including specific content for each container.
- Container list and chemical content detail may be attached as separate document.
