Campus Optics Pickup Request Form (updated 7/28/26)
All chemical waste pickup requests will be transitioning to our Campus Optics platform. You can submit a pickup request by clicking the following link Campus Optics Pickup Request Form, filling out the form, and submitting your pickup request. SSO login is required.
If you are having issues submitting a pickup request via the Campus Optics platform, please use the temporary workaround method listed below to submit your request.

Please follow these instructions to submit a Chemical Waste Pick Up Request:
- Please complete the Word document linked below. Enter chemical information as usual.
- Attach completed form to an e-mail addressed to hwm@ehs.ufl.edu
- Additional information (chemical lists, label information or images) should be attached to the same e-mail.

The Hazardous Waste Pickup Request Forms require GatorLink ID login. If prompted, on the Microsoft account screen, log in as shown below:
GatorLink Username@ufl.edu
On the UF Authentication screen use:
Username: GatorLink Username
Password: GatorLink password
