Will the individual operate a special purpose vehicle?
Special purpose vehicles include industrial or farm equipment such as tractors, forklifts, bulldozers, airboats, etc. In many cases, participation in the Hearing Conservation Program is required. Full Hearing Program details are available on the UF EH&S Hearing Conservation page.
Health Assessment Requirements
The preplacement health assessment for those who will operate a special purpose vehicle includes a preplacement physical examination along with a review of immunization status for tetanus.
- The department:
- Identifies this job duty in myUFL position information or completes the online form Job-Related Health Risk Checklist for Individuals Not on Positions (INOP) based on supervisor input.
- Submits the Payment Authorization Form.
- Calls the UF OCCMED Clinic at (352) 294-5700 to schedule a physical exam appointment.
- The individual: Prior to the exam appointment, completes the Physical Exam and Medical History Form and provides it to Clinic personnel at the time of the appointment.
- The UF Health Hearing Center at the Oaks provides baseline audiograms to UF employees in the Gainesville area when hearing monitoring is required.
- The Clinic: Performs the health assessment and records the result status in myUFL Workforce Monitoring.
