Repeated bending is part of a group of general physical labor job duties that requires a preplacement physical examination. Health Assessment Requirements
The preplacement health assessment for those who will be bending for more than two hours per day includes a preplacement physical examination along with a review of immunization status for tetanus.
- The department:
- Identifies this job duty in myUFL position information, or completes the online form Job-Related Health Risk Checklist for Individuals Not on Positions (INOP) based on supervisor input[cite: 10]. The form is then reviewed by Environmental Health and Safety (EH&S) and the OCCMED Clinic.
- Submits the Payment Authorization Form.
- Calls the UF OCCMED Clinic at (352) 294-5700 to schedule a physical exam appointment.
