Health Assessment Requirements

The preplacement health assessment for those who will have frequent reaching above the shoulder includes a preplacement physical examination along with a review of immunization status for tetanus.

  1. The department:
    • Identifies this job duty in myUFL position information, or completes the online INOP Form based on supervisor input.
    • Submits the Payment Authorization Form.
    • Calls the UF OCCMED Clinic at (352) 294-5700 to schedule a physical exam appointment.
