Pesticide Description
Will the individual be using pesticides?
Based on the type, frequency and amount of pesticide used, pesticide users at the University of Florida may be required to participate in a medical monitoring program. Participants in this program must establish a baseline from blood tests before they begin using the pesticides. They also must take part in annual blood testing. Participants in the pesticide medical monitoring program usually are required to participate also in the Respiratory Protection Program. According to the University of Florida s medical monitoring program for pesticide users, all users* of toxicity class I, II, or III pesticides (oral or dermal LD50 of less than 2200 mg/kg as indicated on MSDS), shall participate in the medical monitoring program for pesticide users.
*”Use is defined as mixing, loading, applying or otherwise handling (except in original, unopened containers) those materials (oral or dermal LD50 of less than 2200 mg/kg) with a frequency of more than 4 days per calendar month (any part of a day counting as one day) and a volume of more than one pint (16 fl oz) of mixed solution or one pound of dry material at any single use.
1. Does/Will the individual use toxicity class I, II or III pesticides (oral or dermal LD50 of less than 2200 mg/kg as indicated on the MSDS)?
2. Does/Will the individual use the pesticides with a frequency of more than 4 days per calendar month (any part of a day counting as one day)?
3. Does/Will the individual use the pesticides with a volume of more than one pint (16 fl oz) of mixed solution or one pound of dry material at any single use.
If ALL answers to the above questions are “Yes” the individual must participate in the Program.
Health Assessment Requirements
The preplacement health assessment for those who will be using pesticides as defined above includes baseline blood testing. In addition, most pesticides users are required to participate in the Respiratory Protection Program that requires a preplacement physical examination along with a pulmonary function test. 1. The department:
- identifies this job duty in myUFL’s position information or completes the on-line form “Job-Related Health Risk Checklist for Individuals Not on Positions” (INOP) based on the supervisor’s input and submits. The form is then reviewed by Environmental Health and Safety (EH&S) and the OCCMED Clinic.
- completes the Review for Respirator Use Form.
- submits the Payment Authorization Form.
2. Prior to the exam appointment, the individual completes the Respiratory Medical Evaluation Questionnaire if required. The individual must bring the form(s) to the appointment.
3. The OCCMED Clinic completes the health assessment and records the result for the individual in PeopleSoft.
