Patient Contact Description

Will the individual have patient contact?

Any employee having physical face-to-face contact with patients or having contact with potentially contaminated items, including (but not limited to) blood and/or body fluids, as a required part of their job duties will be considered to have “patient contact” for the purposes of their preplacement screening by the UF OCCMED Clinic.  Full program details are available from the Student Health Care Center (SHCC).

Health Assessment Requirements

The preplacement health assessment for those with patient contact consists of a review by the SHCC of the individual’s completed medical history and immunization forms.

1. The department identifies this job duty in
   myUFL’s position information or completes the on-line form
   “Job-Related Health Risk Checklist for Individuals Not on Positions” (INOP) based on
   the supervisor’s input and submits. The form is then reviewed by Environmental Health and Safety (EH&S)
   and the OCCMED Clinic.

2.  The department then calls the OCCMED Clinic at (352) 294-5700 for a patient contact review appointment.  

3.  The individual completes and takes to Clinic appointment:

• the Preplacement Screening Patient Contact Form
• the Respirator Medical History Questionnaire
• the Tuberculosis (TB) Surveillance Form

4.  The SHCC completes the health assessment, updates any required immunizations and records the result for the individual in PeopleSoft
