General Safety

q  Notice Board with Emergency Call list and hazard warning labels are posted at lab entrance(s) (provided by EH&S-Lab Safety 392-1591)

q  UF Laboratory Safety Manual is readily accessible

q  A Chemical Spill Kit is available in each lab housing liquid chemicals

q  A First-Aid Kit is readily accessible; Calcium gluconate is available for labs working with hydrofluoric acid.

q  Safety Shower and plumbed Eyewash Unit are readily accessible to lab personnel

q  Airfoils and slots on chemical fume hoods are unobstructed; side panels are in place

q  Storage of chemicals and equipment in chemical fume hoods is kept to a minimum

q  Vacuum pumps are equipped with a belt and pulley guard, if applicable

q  Lab equipment is safely operated and maintained according to manufacturers directions

Personal Protective Equipment (PPE), Housekeeping, and Hygiene:

q  PPE e.g., gloves, safety glasses/goggles, lab coats, thermal protection, etc. is available (stored clean and in good repair) and worn for the activities being conducted

q  Hearing protection is worn for high noise areas (e.g. sonicators, grinders)

q  Full coverage shoes with good sole grips are worn in the lab; no open-toed shoes (sandals) are allowed

q  Lab personnel are aware that contact lenses should not be worn in the labs

q  Respirator wearers are trained, fit tested, and registered with EH&S

q  Walkways and safety equipment are clear of obstructions

q  Work surfaces and benches are free of clutter to reduce risk of spills and accidents

q  Spills are cleaned up promptly

q  Mechanical pipetting devices are used; pipetting by mouth is not allowed

q  Food and beverages are not permitted in the lab working area, or stored in lab refrigerators/freezers

q  Refrigerators, freezers, and microwaves are clearly labeled, e.g., "non-flammable storage only;" "no food or beverages" (provided by EH&S Lab Safety 392-1591)

q  PPE is removed before leaving the lab

 
Chemical Safety:

q  Proper storage procedures are followed for all chemicals including labeling, capping/sealing, and proper container type/condition

q  Liquid chemicals are stored below shoulder height

q  Proper dating/storage/use/disposal procedures are followed for perchloric acid

q  Proper dating/storage/use/disposal procedures are followed for peroxide forming compounds


Lab waste disposal:

q  Lab personnel are trained in EH&S Hazardous Materials Management and training dates are available on EH&S Training Calendar

q  Outdated and unused chemicals are disposed of through EH&S Hazardous Materials Management

q  All hazardous waste containers are properly sealed and labeled with yellow EH&S labels

q  A Hazardous Waste Satellite Accumulation Area is designated and the requirements are posted (last page of the Chemical Waste Management Guide)

q  Biological Waste is decontaminated and disposed of properly

q  Broken glass is separated from regular waste stream and properly labeled as noted in University of Florida Policy for Disposal of Clean Lab Ware

q  Needles and small sharps are disposed of in red plastic ‘sharps’ box


Compressed gases:

q  Compressed gas cylinders are adequately secured (even when empty) and stored by compatibility

q  Gas cylinders are securely transported using a hand truck

q  The UF Compressed Gas Rules are posted in a prominent location

q  Cylinders are stored away from heat sources

q  The regulator connection is leak tested after installation and before each use

q  Cylinders with no regulators must be capped (even when empty)

q  Contents of cylinders must be clearly labeled

q  Hydrostatic tests are current (cylinders have not been stored more than 5-10 years

Electrical Safety:

q  Access to circuit breaker panel is unobstructed

q  Openings on breaker panel, receptacle boxes, etc. are sealed

q  Ground Fault Circuit Interrupters (GFCI) are used near sinks and wet areas

q  Electrical cords are checked periodically for fraying or damage

q  Lab appliances are plugged directly into electrical outlets (UL rated power surge protectors may be used when there is a lack of electrical outlets but extension cords cannot be used as a permanent power supply)

Fire Safety:

q  A Fire Extinguisher is located near or in the lab

q  Large metal drums of flammable liquids are grounded during transfer

q  A maximum of 10 gallons of flammable liquids are stored in a lab; the excess is stored in a safety cabinet or safety cans up to a maximum total of 50 gallons

q  Flammable liquids over 1 gallon are stored in safety cans

q  Sprinkler heads are not blocked

q  Vents on flammable storage cabinets are sealed


Drug Enforcement Agency
(DEA) controlled substances:

q  DEA permit is current (A DEA permit is required for controlled substances)

q  DEA use log and employee questionnaires are current for controlled substances on hand

q  Controlled substances are stored in secure location

q  Outdated or unwanted DEA substances are disposed of appropriately (contact EH&S Hazardous Materials Management for more information)

q  Bio-Safety Level (BSL) of 2 or 3

q  State or federally regulated materials

q  Cell lines or cultures that have been immortalized with a virus or are primary human tumor cells

q  Human blood or other tissues that are known to be HIV positive (or positive for any human disease-causing virus or other agent), when used in research

q  Blood or other tissues from old world monkeys

q  A recent (within the last year) chemical inventory must be kept on file (email to labsafety@ehs.ufl.edu on annual basis)

q  Bloodborne Pathogen Program/Exposure Control Plan

q  Appropriate safety manuals e.g., UF Laboratory Safety Manual, UF Biological Safety Manual, UF Chemical Waste Management Guide, etc.
