Documents that viewers may find useful in submitting proposals:

The Suggested Format for New Proposals to Use Radioactive Material to the Radiation Control Committee (RCC) must be completed by each Principal Investigator who wishes to use radioactive materials for non-human use research.  The Summary Sheet Proposal to Use Radioactive Material should also be submitted.

The Statement of Training and Experience form must be completed by all personnel who will be working with radioactive materials.  If the RCO deems the training and experience inadequate, a Documentation of Training form will be forwarded by the RCO and must be returned.

The Renewal of Radioactive Material Use Authorization form is required on a two year frequency and will be sent from the RCO prior to expiration of current approval.

 

The Application for the Use of Radionuclides in Human Subjects to the Human Use of Radioisotopes and Radiation Committee (HURRC) must be completed by each Principal Investigator who wishes to use radioactive materials for human use research.  This application can also be located in the Institutional Review Board (IRB) Investigators Manual as Appendix D.  The appropriate risk statement for the Informed Consent Form must also be included.

The Human Use of Radioisotopes and Radiation Committee (HURRC) Human Use Protocol Renewal Report form is required on an annual frequency and will be sent from the RCO prior to expiration of current approval.

The Application for the Use of Radionuclides in Human Subject to the Radioactive Drug Research Committee (RDRC) must be completed by each Principal Investigator who wishes to use radioactive drugs for human use research.  This application can also be located in the Institutional Review Board (IRB) Investigators Manual as Appendix F.  The appropriate risk statement for the Informed Consent Form must also be included.
