When individuals require a
preplacement health assessment before they begin work, the department must
initiate and track the process with the Student Health Care Center (SHCC).
1. The department identifies
the job duty that requires the health
assessment. Job duties that require a health assessment
can be found at
http://www.ehs.ufl.edu/OCCMED/jobduty.pdf.
For individuals who will fill a specific position at
the
For individuals who will
work without a specific position (OPS or volunteers), the department will
record the job duty on the new Environmental Health and Safety (EH&S) form
at http://www.ehs.ufl.edu/formium/server.asp?form=INOPOn.pdf. The department then submits the form to
EH&S/SHCC.
2. The
department initiates the health assessment with the following information.

Candidate’s
name
·
Candidate’s
department
·
Candidate’s
UFID number (Employee ID) – if the UF Directory does not list the individual’s
UFID, the individual must be added to the directory
·
Candidate’s
position title and number (if applies)
·
Supervisor’s
name and phone number
The type of health assessment depends on the job duty.
The
department prompts the individual to complete and submit the appropriate forms
if the following job duties apply.
Delivery of the above forms to the SHCC can be in person at the main
campus

Note: This requires an appointment with a nurse at
the SHCC Occupational Medicine Clinic.
B. On-site
physical exam and review of immunization records
The department calls the
SHCC at (352) 392-1161 x 4212 for an appointment for the following job
duties. The department may also call the
SHCC at Shands in the Health Science
Complex at 392-0627 for some appointments.

The department calls the Speech and Hearing Clinic at (352) 392-2041 for a baseline audiogram for the following job duty.
· Work in areas of excessive noise
3. The SHCC records the health assessment result and date on the PeopleSoft Physical Exam screen.
4. The department checks the PeopleSoft Physical Exam screen for the appropriate review status (Navigation: Workforce Monitoring, Health & Safety, Record Medical Exam Results, Physical Exam). The status date, just as the current HAMS date, must be before the individual begins work.
