Health Assessments for Individuals Not On Positions

 

Environmental Health and Safety provides a form for departments to request a preplacement health assessment for employees hired for individuals not on positions.  This includes Other Personnel Services as well as volunteers.

 

The Job-Related Health Risk Checklist for Individuals Not On Positions at http://www.ehs.ufl.edu/formium/server.asp?form=INOPOn.pdf includes the same job duty list that is in PeopleSoft but is for those who will be hired without a specific position.  The department completes the form on line and submits it as an attachment to email to Environmental Health and Safety.  The completed form may also be saved to the submitter's computer using only the Acrobat Reader software. Once reviewed, EH&S then sends it to the Student Health Care Center.
