EMPLOYEE HEARING CONSERVATION PROGRAM (HCP)

1. Audiograms must be provided by or supervised by a Certified Clinical Audiologist. If a technician performs the testing, he/she must be responsible to an audiologist.
2. Procedures:
a) Employee given “Hearing Screening Medical Form” questionnaire for completion. The completed form is brought to the audiologist at the time of the appointment.
b) Completed questionnaire reviewed by audiologist and reviewed with patient
c) Otoscopy performed, and results recorded
d) Audiogram performed per OSHA criteria, pure tone, air conduction, hearing threshold exam with test frequencies of 500, 1000, 2000, 3000, 4000 and 6000 Hz.
e) Discuss results with employee and note need to return in one year.
f) If a medical referral is made, the referral must be documented in a letter to the individual with a copy sent to UF’s Student Health Care Center (SHCC) for inclusion in the employee’s medical record
g) If an OSHA defined STS is identified, the employee will return within 30 days for a retest.
h) Individual audiograms along with interpretations will be sent to the SHCC for inclusion in the employee’s medical record.
i) A summary of all the audiograms with interpretations and recommendations will be sent to Environmental Health and Safety (EH&S). A sample summary form is included.
