
The University of Florida provides periodic medical monitoring as part of the Occupational Medicine Program. The monitoring is after the preplacement or initial health assessment for individuals who perform selected job duties that require health assessments at varying periods of time. Job duties that involve various types of follow-up are listed below. Descriptions of the job duties and any related programs may be found in the linked files.
OBTAINING A PERIODIC HEALTH ASSESSMENT
Environmental Health and Safety (EH&S) tracks individuals in the Periodic Medical Monitoring Programs. Each program has its own specific frequency at which time the participating employee must obtain a follow-up health assessment. If you are unsure of when an individual is due for a follow-up, please contact EH&S at (352) 392-1591.The Student Health Care Center (SHCC) provides periodic medical monitoring for employees in the Gainesville area in their Team C Clinic. For a periodic health assessment appointment, please call the SHCC at (352) 392-1161 x 4212. Appointments for periodic follow-up are available on Tuesday afternoons.
The SHCC provides immunizations for employees on a walk-in basis in Team C Monday afternoons from 1:00-3:00 p.m. and Tuesday mornings from 8:00-11:30 a.m. Student Health at Shands also provides immunizations for employees. Please call them at (352) 392-0627 for available hours.
Follow-up appointments for Workers' Compensation injuries are available Thursday afternoons in Team C. (Please remember that acute Workers' Compensation cases will be seen in Team E as needed.)
The Speech and Hearing Clinic within the Department of Communication Sciences & Disorders provides audiograms for UF employees who work in areas of excessive noise. Annual audiograms may be scheduled by calling at (352) 392-2041. Their clinic is located in Dauer hall, directly east of the Student Health Care Center.
Many of the periodic medical programs have forms that require completion by the employee and the supervisor. Please refer to the program forms list that is in order by job duty.
For employees outside the Gainesville area, periodic health assessments are usually obtained from the same provider who offered the preplacement health assessment. As with these first time assessments, the medical records from the periodics should be faxed to the SHCC for review.
PAYMENT OF A PERIODIC HEALTH ASSESSMENT
Payment of all periodic health assessments is the responsibility of the employee's department or college.
