Chemical Waste Pickup Request Form

The Chemical Waste Pick-up Request Form has been modified from the 3-part form (EH&S/RSA-1 11/92) to a standard 8 1/2" x 11" one-part form (EH&S/RSA-1 5/99).

Or, you can download the form here as an Adobe Acrobat file and print or copy as many as you need. If you use Acrobat Reader version 3.01 or above, the form may be filled out on-screen in the Acrobat reader. Once the form is filled out, print it and send it by campus mail to Box 112725.
