Controlled Substances Disposal

Disposal Policy:

All DEA controlled substances must be accounted for upon their disposal.  Expired or unused controlled substances may be disposed of through Universal Solutions Inc.  Please see instructions and necessary forms to be completed below.  A current certification of registration will be required.  Keep all disposal records for at least two years.

Disposal Instructions:

1.   Complete appropriate UniPAK Express Application form (for disposal of either schedule C-II or schedule C-III, C-IV, & C-V controlled substances).   These Acrobat forms may be printed and filled out by hand or filled out on your computer and then printed.   The original packaging should identify the “C” schedule number.  If this cannot be found on the original packaging, refer to the DEA Briefs & Background, Drug Policy, Drug Scheduling web page to help identify your material.
 

2.   Send signed, completed UniPAK Express Application form(s), a copy of your DEA certification and a copy of the memo which authorizes payment (using our  P.O. agreement with Universal Solutions) to Universal Solutions.  DO NOT send the controlled substances at this time.
 

3.   Universal Solutions will send shipping instructions along with a shipping label. 
 

4.  Package materials following the specific instructions provided and ship to Universal Solutions.
 

5.   Universal Solutions will confirm receipt of the material and of its eventual destruction.
 

6.   Provide copies of original express application and confirmation of destruction to this office (fax # 392-7286 attn: Matt Doty)

 

The forms are straightforward but a few instructions might be helpful:

Use the information from your DEA registration to complete the “Applicant Information” section.  Product name and strength refers to the original strength of the product (i.e. mg/ml not current dilution).  NDC numbers can be found on the product container (in microscopic font unfortunately) or online at the following locations:

“Orphaned” Controlled Substances:

Controlled substances left over from previous laboratory occupants, retired investigators, or investigators no longer working at the University of Florida must be handled as “Orphaned” materials.  Laboratories/Investigators in possession of substances identified as controlled substances without current DEA certification to possess those substances are in violation of the Controlled Substances Act and must dispose of these materials as soon as possible.  Materials identified as controlled substances should be kept in a secure location (e.g. in a safe, lockbox, or lockable cabinet).  Please contact the Chemical Waste Management Facility for disposal information at (352) 392-8400.
