Individual Diver Log or Project Dive Log
If a project uses more than two people on a dive, this log provides the
tender/divemaster/lead diver with one log which may be completed as the dive progresses
and subsequently submitted by the fifth of the following month to insure that all
dives are logged and each diver is accurately represented. Note that there is a place for
each diver and the diver master to sign/initial. This may be sent by mail, or by fax.
Personal logs such may also be used. However, for simplicity of completion the project log
permits one person to complete the log, copy it for personal/project files, and submit it
to the DSSP. Please add the Project Number to the log.
